Autumn 2021 New Student Checklist

Incoming MS and PhD students in the Physical Sciences Division should complete the following tasks. The relevant deadlines are included below.

MS Students

Incoming MS students should complete the following: 

Submit proof of degrees

Deadline: September 15

If you have not done so already, please submit proof that you received degree(s) from your prior institution directly to your program. The deadline to submit these materials is September 15, 2021.

We understand that some of you may not receive proof of degrees from prior institutions until after September 15, 2021. If this applies to you, please contact your Student Affairs Administrator to let them know when they can expect to receive your proof of degrees.

Complete immunization proof requirements

All students should have uploaded proof of COVID-19 vaccination (or an applicable exemption) by September 1. Answers to frequently asked questions are available on the GoForward website.

If you are still in the process of being vaccinated or have any questions about the vaccine requirement, please make sure you have contacted studentwellness@uchospitals.edu.

Students who need to receive COVID-19 vaccination at the University of Chicago must email studentwellness@uchospitals.edu with their phone number, 8-digit student ID number, and the date you anticipate arriving on campus.

Students who prefer to receive their COVID-19 vaccine off campus should email a copy of their vaccine appointment confirmation to studentwellness@uchospitals.edu

Students must also comply with the non-COVID-19 immunization requirements. Due to the large number of inquiries related to the COVID-19 vaccine requirement for Autumn Quarter, the University is prioritizing review of COVID-related documentation. The process for non-COVID immunizations will not interfere with students’ ability to participate in Autumn Quarter activities as long as the COVID-19 vaccine requirement is met.

Complete COVID-19 Attestation

Deadline: September 27

All members of the University community, whether they will be on campus or not, are required to complete the new COVID-19 health requirements attestation. The annual attestation must be completed by September 27, 2021, the start of the Autumn Quarter.

Enroll/Waive University Student Health Insurance Program (U-SHIP) (MS Students only)

Deadline: October 15

All MS students who are eligible for U-SHIP must confirm enrollment in U-SHIP OR waive U-SHIP by submitting proof of comparable coverage. The deadline to do so is October 15, 2021. More information is available on the insurance website.

If you are eligible for U-SHIP and do not take action by October 15, 2021 will be automatically enrolled in U-SHIP for the 2021-2022 academic year and be responsible for the quarterly insurance premiums.

Responsible Conduct of Research (RCR) Training – Statistics MS students only

Statistics MS students must complete and obtain certification in the Responsible Conduct of Research through the Collaborative Institutional Training Initiative (CITI). Please review the instructions on the PSD’s website. The instructions link directly to the training, and you will complete CITI training entirely online.

Statistics MS students must complete the CITI program RCR training and earn certification by the end of your first quarter. We strongly recommend you complete your training prior to the start of Autumn Quarter (September 27). The training will ensure you are better prepared for your ethical responsibilities as academic researchers.

Sexual Misconduct Awareness and Prevention Training
You will receive an invitation to complete the sexual misconduct awareness and prevention training. This training is mandatory for all members of the University of Chicago community, including students, faculty, other academic appointees, staff, and postdoctoral researchers. You must complete this training by the end of your first quarter.

Annual Confirmation Task
Each year, new and returning students must complete their annual confirmation task to ensure we have up-to-date records of contact information. Instructions for completing your Annual Confirmation Task can be found on the Registrar’s website.

Submit a Photo for your UChicago ID Card
Students should submit a photo of themselves for their UChicago Card (the ID card you use to access campus buildings and resources) by visiting https://myphoto.uchicago.edu/.

UChicago Cards will be available for pick-up by appointment at the ID & Privileges Office.

PhD Students

Update Address and Direct Deposit Information

Deadline: September 15

PhD students should update their addresses in my.uchicago.edu and Workday and add direct deposit information in Workday. Instructions for completing those updates are below:
